docs: 明确 OSI 访问为 host+proxy 组合及代理语义
...
- socks5_proxy 扶正为必填（OSI 主机在内网、不可直连，经 SOCKS5 到达）
- 作用域限定：代理仅用于 OSI 客户端，PHIS 走直连
- 访问语义：非空即强制走代理、不因代理故障回退直连，空才直连；
代理不通按网络错误如实失败，交重试/熔断处理

按团队 AGENTS.md 的扁平偏好，弃用 internal/+cmd/，改为顶层平铺包 +
单 main.go(-mode server|deliver)。同步更新 docs/03 目录结构图与 §4.4、
docs/05 路线图措辞、CLAUDE.md 目录职责表。

- 随项目留存厂家 OSI 规范源材料（docx/xlsx，内部保密，仅限本私有仓库）
